Exclusion-Applicants who meet the following criteria will be excluded


  1. Do not meet the requirements set forth in the preliminary assessment process.
  2. Do not have mild to moderate mental retardation.
  3. Are under the age of 18.
  4. Are not able to benefit from services.
  5. Cannot be accommodated by WCRS due to certain clinical or medical needs.
  6. Have not completed required paperwork within specified time frame.
  7. Are not free from communicable diseases (ex. Tuberculosis).
  8. Are not eligible for Medicaid Waiver through a community services board or private funding.
  9. Have hearing impairment that requires communication through sign language.
  10. Have a record of violent behavior.
  11. Pose a threat to his/herself or to others.
  12. Require continuous nursing care (around the clock observation, assessment, monitoring, supervision, or provsion of medical treatment by a licensed nurse).
  13. Are certified by their physician that placement is no longer appropriate.



Levels of Intellectual Disabilities (ID)



Individuals with mild "ID":


  • are likely to need only intermittent to limited support;
  • typically do not "look" different from their non-disabled peers
  • often have only mild or moderate development delays, except in academics, which is often the major area of deficit;
  • they typically attain 3rd to 6th grade academic achievement levels by the time they finish high school;
  • as adults, many , though not all, with mild ID will be able to obtain independent employment;
  • many will marry, have children, and blend rather indistinguishably into the community; for those who achieve total independence, the label of ID is no longer appropriate.

Who We Service:



We Care Residential provides quality care and service to adult males and females ages 18 years of age and older with mild to moderate intellectual disabilities who are not ready for independence. Intellectual disability or "ID" was once referred to as mental retardation. Any of various conditions characterized by impairment of an individual's normal cognitive, emotional, or behavioral functioning, and caused by social, psychological, biochemical, genetic, or other factors, such as infection or head trauma.

Individuals with moderate to severe "ID":


  • will probably need limited to extensive supports;
  • they are more likely to have a recognizable syndrome (such as Down Syndrome);
  • therefore, may "look" different than their non-disabled peers;
  • their development is often significantly delayed;
  • they are typically identified as infants or toddlers;
  • most begin receiving special education during the preschool years;
  • as adults, most individuals with moderate to severe ID will not achieve total independence;
  • rather, they are likely to continue to need limited to extenive support such as that provided in group homes or semi-independent living situations (SILs); some may continue to live with their parents
  • some individuals with moderate to severe ID may be able to succeed in modified competitive employement situations;
  • however, many will work in supported, non-competitve settings usch as sheltered workshops.







Continued Stay- Applicants must meet the following   criteria:


  1. Meet the requirements and conditions set forth in the preliminary assessment.
  2. Show measurable progress from ISP goals.
  3. Are able to follow the rules and regulations of the residence.
  4. Pose no safety threat to themselves, other residents, or staff.
  5. Have develop no illness that requires extensive nursing care.

Admission


Applicants must meet the following criteria


  1. Have mild to moderate intellectual disability.
  2. Be 18 years of age or older.
  3. Are eligible for Medicaid Waiver through a community services board or private funding.
  4. Have the ability to benefit from services provided.
  5. Are willing to participate in appropriate training.
  6. Have a current physical examination, including statement from their physician verfying that they are free from communicable diseases (ex. Tuberculosis).
  7. Have all admissions paperwork and physicals completed.
  8. Have no hearing impairment that requires communication through sign language.
  9. Have medical and clinical needs that can be accommodated by WCRS personnel and/or contractors.
  10. Are willing to participate in community experinces provided in individual and group settings.
